Proxy Access

Proxy Access is when patients choose to give another person access to their online GP record. This is usually a family member, next of kin or a carer.

A proxy may be chosen for many reasons, including:

  • Providing better information to family members and carers about your medical conditions
  • Giving someone access to your prescriptions record, so they can understand what medications you need and how much.

To request proxy online access:

  • collect a proxy online access form from reception

The NHS website has information about using linked profiles to access services for someone else

The NHS App needs both people to be registered at the same GP Practice.

If the possible proxy is not registered at our GP Practice and our patient is an adult, we can set up online proxy access via Patient Access.
